get online at your bank you can do it there. it is very simple;
Write it in formal letter format, to do so, heres an Example, just replace it with your information and the rest of the information needed(Replace most of the text).

Second, you would need to explain it to them within the body.
Third, end with Sincerely, or Best wishes, or whatever it is you may see fit. And of course restate your name and whatever else you wish to include.
However, depending on your bank account and who your with(Wells Fargo, Bank of America, etc…) Some of them may already have it via the internet, all you need to do is find the bank your with, and it’s website.
Google the website, and see if you can apply via online with them, if you cannot find it, or w/e, feel free to write the letter directly towards the nearest bank, or you may even go to them directly and wait in line to give it to them.
